JAG Physical Therapy Appointment

Please bring to your first JAG Physical Therapy appointment:

  • Proper forms of identification
  • Insurance cards
  • Payment information
  • Doctor’s referral
  • Loose fitting clothing (This allows physical therapists to treat the injured area)

Each treatment can last anywhere from thirty to sixty minutes, depending on the techniques and treatments utilized.

Communicate any pain encountered during or after treatment to the physical therapist so they can make proper adjustments and create a customized treatment plan designed specifically for your injury and to help reduce pain and provide relief.

    Payment FAQs

    Why do I have to pay at the time of my visit?

    The amount collected at check-in is an estimate based on the insurance information available at that time.

    Is the amount I pay at my visit my final balance?

    No. Your final balance is determined after your insurance carrier processes the claim.

    Can I still owe money if I have secondary insurance?

    Yes. Secondary insurance does not always cover the full remaining balance, so you may still be responsible for part of the cost.

    What charges might I still owe?

    Depending on your coverage, you may still be responsible for your deductible, copayment, coinsurance, or other out-of-pocket expenses.

    What should I do at check-in?

    Please provide complete and accurate insurance information at check-in. If you have Medicare, Medicaid, or secondary coverage, let the front desk know so your coverage can be reviewed correctly. This matches the payment notice, which explains that the visit payment is only an estimate and that final responsibility is determined after insurance processing.
